Do You Believe Someone Filed for PUA Benefits in Your Name? Learn about Reporting PUA Fraud
If you suspect or know that someone is using your information to file for benefits, you can report them to the Department of Labor and Industry:
- Online: You can complete and submit the Identity Theft form on DLI's website here: https://www.dlisecureweb.pa.gov/FRTS/IdentityTheft.aspx
- By Phone: You can call the PA Fraud Hotline at 1-800-692-7469
If you received a check for PUA in the mail
- Do not cash the check
- Return the check(s) to this address:

Harrisburg, PA 17120 - Include a brief signed statement with the reason you are returning the check with your printed name, address, last four digits of your social security number, phone number and email address.
If you received a UC-issued debit card in the mail
- Do not activate the card
- Email a picture of the back of US Bank ReliaCard or the number on back of the card, not the front, to officeofinvestigations@patreasury.gov

- Return the US Bank ReliaCard by mail to:

If you have tried to file a PUA claim for yourself and received a message that there was already an account associated with your Social Security Number, there may be a fraudulent account. You should follow the identity theft instructions above. You should also email UCPUA@pa.gov and let them know you reported the identity theft and need help setting up a real account in your name.
